Community Health Nurses and Directors

Health Canada has commissioned Ference Weicker & Company, a management consulting firm, to conduct an evaluation of the Communicable Disease Control (CDC) Cluster within the First Nations and Inuit Health Branch. The CDC Cluster relevance and effectiveness evaluation includes three program areas: Vaccine-preventable Diseases (Immunization); Airborne and Respiratory Diseases (Tuberculosis); and Blood-borne Diseases and Sexually Transmitted Infections (HIV/AIDS).

A key part of the evaluation is obtaining input from community participants and community health nurses and directors. Your participation is critical to assess the relevance and effectiveness of the CDC Cluster in contributing to the improved health status of First Nations people and Inuit within your community. The findings of this evaluation will be summarized in a final report to be utilized by Health Canada in improving its programs and services.

The opinions and individual feedback from participants will be kept confidential and will remain anonymous throughout the entire process. We will not share the individual responses with any other parties.  All information you provide will be kept confidential and only aggregate data will be used in our report.

It should be noted that all information under the control of Health Canada is subject to both the Access to Information Act and the Privacy Act.  The following describes certain obligations under these statutes that we must tell you.  The information collected will be retained in accordance with the Retention and Disposal Schedule and then destroyed (or sent to Public Archives, if applicable).  We can assure you that any identifiable personal information will be protected in accordance with both Acts. The information you provide will only be used for the purpose of this evaluation and will not be disclosed to any external third parties, except as specified by the Privacy Act.  In very limited and rare circumstances, personal information may be disclosed.  Any questions about these two laws can be directed to the Access to Information and Privacy Division at (613) 954-8744.
